Financial Policy
Thank you for being our patient
We are committed to building a successful physician-patient relationship with you and your family. Your clear understanding of our financial policy is important. This information was designed to provide our patients with a detailed explanation of our financial policies. We realize this information may not apply to your specific situation and encourage you to please speak to our staff with any questions or concerns that you may have.
ID and Insurance Card
Photo ID and insurance cards must be presented at every visit. We will be unable to see a patient without a valid photo ID. This is for identity protection. We will copy your insurance card periodically to keep our files current. Please help us to ensure the information we have is current and accurate and know what your insurance contract will cover and pay so you are not subject to unexpected costs.
Co-Pay, Co-Insurance, and Deductibles
Remember we are here to help you. Your insurance company requires us to collect your co-pay at the time of service. Waiver of co-pays may constitute fraud under state and federal laws. We ask that you pay your co-pay at each visit as well as any deductibles, fees for non-covered services and any other cost shares you may incur. If you are unable to pay at the time of service a $5 fee will be added to your account as a statement fee.
Patient statements are expected to be paid in full promptly upon receipt. Past-due balances that go beyond 90 days will be handled by an outside agency. We are willing to work with you to help avoid this occurrence. Patients whose accounts are sent to collections will be required to pay all outstanding balances, including any collection agency fees, before scheduling any future appointments.
Self-Pay Patients
If you do not have a group or individual medical policy, payment is expected in full at the time of service.
Returned Checks
Patients whose checks are returned by the bank for insufficient funds will be charged a $35 fee and future visits will need to be paid in cash or credit.
Cancelations
Kindly give our office 24-hour notice if you need to reschedule your appointment. Patients who do not show for their scheduled appointments will be charged a $50 fee that must be paid prior to receiving any further services.
Payment Plans
At Oakland Eye Care, we kindly ask that all payment plans be arranged with our billing office in advance of your appointment. To ensure smooth service, please adhere to the agreed payment schedule. Accounts with overdue payments may be referred to an external agency for further management.
Medical vs. Vision Insurance
If you are seen in our office for a medical condition, such as glaucoma, cataracts, macular degeneration, diabetes or dry eyes, your exam will be billed to your medical insurance. If you do not have a medical condition, your routine eye exam will be billed to your vision insurance. If during your routine eye exam, the doctor finds a medical condition, your future exams will be billed to your medical insurance.
Payment Options
For your convenience, our office accepts cash, check, VISA, Master Card, Discover, American Express and Care Credit.
Referrals
Many Insurance plans require a referral/authorization for the services performed in our office. You will need to obtain this referral/authorization from your primary care physician prior to being seen in our office.
Minor Patients
Any adult, who brings in a minor patient 17 and under, will be responsible for any payments due at the time services are rendered regardless of any divorce decrees. If a responsible adult is not present, the appointment will need to be rescheduled.
Refractions
Refraction is the process of determining your eyeglass/contact prescription by asking you what lens you see better with (i.e. better 1 or 2 or are they about the same?) Although this is important to helping you see better, most health insurance companies do not pay for this and the fee, of $40, is due at the time of service.
Eyewear/Contact Lenses
We ask that you pay in full when you place an order for contact lenses, contact lens supplies or optical accessories. Eyeglass orders require you pay half when ordering, with the balance due a the time of pickup.
